“Lessons Learned from Historical Project Fails”

 

Synopsis: When major historical project failures occur, it is easy to assign the event to an “act of god” or to a single-point of failure.  However, many of these events occur through a complex combination of events based upon project decisions made sometimes years earlier.  This presentation will look at three historical project failures:  The Challenger Shuttle disaster, the British Comet Jet crashes and the Tacoma Narrows Bridge destruction.  For example, did you know that engineers prior to the Challenger launch stated that a disaster would be imminent if launched?   By examining how project management decisions played a part in these three historical events, we can understand some blind spots which lead to project failures.  As it is said, “History doesn’t necessary repeat itself, but many times it rhymes.”
